Wraparound

Mission
The Wraparound program is designed to enhance the scope of community-based Mental Health/Mental Retardation services for children and their families in Bradford and Sullivan Counties. Our goal is to provide specific outcome-monitored services to children and adolescents as part of an integrated service delivery system.

Wraparound utilizes individualized strength-based assessments to treat disorders diagnosed in the DSM IV. We utilize an inter-agency team to develop goals for a child or adolescent, while supproting and helping to maintain a child in the community.

Wraparound services can also be used to increase the effective transition of children returning to their home frm an out-of-home placement.

Wraparound services are delivered in the child's home, school, or other community setting. Examples of services available include Psychological Evaluation, Therapeutic Support Staff, Mobile Therapy, and Behavior Specialist Consultation.

Services
Our services include:

  • Home/ school support and skills training
  • Behavioral Modification
  • Availability 24 hours/ 7 days a week, if needed

Wraparound services are designed to address the needs of children and adolescents under 21 years of age who are eligible for Medical Assistance benefits.

Wraparound services must be deemed medically necessary by a psychiatrist or licensed psychologist. The evaluation and interdisciplinary team determines duration and frequency of services.

Referrals are accepted from individuals, families, schools, Bradford and Sullivan County agencies, providers of children and adolescent services, and other referral sources.
